Physical Information Fact Sheet for Parents/Guardians
READ THIS SECTION IN ITS ENTIRETY
Prior to participation on a school-sponsored athletic team, every student-athlete in grades 6 through 12 must complete the following forms:
- Preparticipation Physical Evaluation History Form – to be completed by the parent & student-athlete prior to the physical exam. This form should be maintained by the healthcare provider completing the physical examination.
- Preparticipation Evaluation Physical Examination Form – to be completed by the student-athlete’s healthcare provider. This form should be maintained by the healthcare provider completing the physical examination.
- Preparticipation Physical Evaluation Medical Eligibility Form – to be completed by the student athlete’s healthcare provider. This is the only form signed by the student’s healthcare provider that should be submitted to the Athletic Department.
- New Jersey Department of Education Health History Update Questionnaire – to be completed and signed by the parent/guardian of the student athlete. This should be submitted to the Athletic Department prior to the start of every season.
Once submitted to the school, the Medical Eligibility Form will be kept in the student’s health file and recorded on the A-45. Pursuant to N.J.A.C. 6A:16-2.4, 2.2(h)1iv, and 2.3(a)4iv, each school district is required to “provide to the parent written notification signed by the school physician stating approval of the student’s participation in athletics based upon the medical report or the reasons for the school physician’s disapproval of the student’s participation.”
NOTE: PLEASE be aware that the school physician must review and approve ALL Preparticipation Physical Evaluation Medical Eligibility Forms. If the Preparticipation Physical Evaluation Medical Eligibility Form is handed in late, the official confirmation for participation on an athletic team can be delayed, regardless of the status determined by your personal physician. Therefore, it is important to obtain the examination in a timely manner. Any missing information will result in disapproval and will also delay the athlete’s participation.
The following forms need to be completed prior to the first sport a student participates in the Genesis Parent Portal:
- concussion sign-off,
- steroid sign-off,
- sudden cardiac death sign-off, and
- opioid sign-off.
